What is the purpose of the AF IMT 102 form?
The main purpose of this form is to register a personal complaint relating to individual injustices or suspected fraud, waste and abuse. The information provided in the complaint is furnished to the airman’s supervisor, commander or inspector. These data is very important for the resolution of the complaint. The form contains the personal information about the individual and the detailed description of the allegations.
What documents must accompany the AF IMT 102 form?
As a rule, this form is not accompanied by other documents. The complainant may attach the supporting evidence of the allegations.
When is the Registration form due?
The complainant must complete and send the form as soon as possible after the first fraud, waste and abuse happened.
What information should be provided in the AF IMT 102 form?
The complainant completes the following clauses:
- Personal information (name, grade, race, sex, social security number, address, home and work telephone number)
- Description of allegations (the complainant can use additional sheets)
- Names and positions of the witnesses
- The form should be as well certified and dated.
The second section is completed by the inspector general staff once they receive the complaint. They have to indicate the file reference number; office symbol; date of opening and finalizing; number of processing days; complainant status; passcode of the complainant; description of the taken corrective actions.
What do I do with the form after its completion?
The airman sends the form to the corresponding inspector general office.
